Ankle Brachial Pressure Index: An update for the vascular specialist and general practitioner.
CONCLUSIONS: A thorough understanding of the correct technique and associated limitations of ABPI measurement is essential in accurately generating and interpreting the data it provides. With this knowledge, the ABPI is an invaluable tool to help manage patients with peripheral arterial disease. Perhaps more importantly, ABPI can be used to identify and risk stratify patients with asymptomatic peripheral arterial disease, itself a major indicator of significant underlying cardiovascular disease. With the emergence of best medical therapy, targeted pharmacotherapy and lifestyle changes can reduce the risk of major cardiovas...

Risk of incident stroke in patients with Alzheimer disease or vascular dementia
Conclusions: Patients with VD, but not AD, have a markedly higher risk of developing an ischemic stroke than those without dementia. In patients with AD, but not VD, use of atypical antipsychotic drugs was associated with an increased risk of TIA.

Risk of vascular events in patients with polymyalgia rheumatica.
Abstract BACKGROUND: Polymyalgia rheumatica is one of the most common inflammatory rheumatologic conditions in older adults. Other inflammatory rheumatologic disorders are associated with an excess risk of vascular disease. We investigated whether polymyalgia rheumatica is associated with an increased risk of vascular events. METHODS: We used the General Practice Research Database to identify patients with a diagnosis of incident polymyalgia rheumatica between Jan. 1, 1987, and Dec. 31, 1999. Patients were matched by age, sex and practice with up to 5 patients without polymyalgia rheumatica. Patients were fol...

Vascular Burden and Cognitive Function in Late-Life Depression.
CONCLUSION: In depressed older persons, vascular burden is related to slower processing speed also in the absence of manifest vascular disease. Poorer interference control was only related to vascular risk factors but not to subclinical or manifest vascular disease. PMID: 25091518 [PubMed - as supplied by publisher]

GoogleFOAM
The expansion of the FOAM world is a bit like blowing up a balloon. The early stages involved a lot of effort for a small increase in size, but as it gets bigger, the resistance is falling away and expansion is getting faster and faster. Obviously this is a good thing. But it can make it easier for us all to get lost and there is the danger of information overload — indeed, I’ve written a guide to (in)sanity in the age of FOAM that I continually update, the LITFL post on Information Overload. The FOAM world is now impossible to keep up with. We need to be able to find what we want when we need it, according to our ...

Increased risk of vascular disease associated with gout: a retrospective, matched cohort study in the UK Clinical Practice Research Datalink
Conclusions In this cohort of over 50s with gout, female patients with gout were at greatest risk of incident vascular events, even after adjustment for vascular risk factors, despite a higher prevalence of both gout and vascular disease in men. Further research is required to establish the reason for this sex difference.

Developing a computerised search to help UK General Practices identify more patients for palliative care planning: a feasibility study
Conclusions: It is possible to significantly improve the identification of patients for palliative care needs assessment using a computerised search however barriers remain to GPs’ finding it acceptable. Time-efficient systems were important as was a generic tool for anticipatory care planning not linked to ‘palliative’ care.

Cognitive performance in depressed older persons: the impact of vascular burden and remission. A two‐year follow‐up study
ConclusionsOur findings suggest that cognitive deficits in depressed older persons are not just a manifestation of depression. In addition, vascular burden was not associated with worse cognitive decline in a sample of depressed older persons.

Adding statistical regularity results in a global slowdown in visual search.
Abstract Current statistical learning theories predict that embedding implicit regularities within a task should further improve online performance, beyond general practice. We challenged this assumption by contrasting performance in a visual search task containing either a consistent-mapping (regularity) condition, a random-mapping condition, or both conditions, mixed. Surprisingly, performance in a random visual search, without any regularity, was better than performance in a mixed design search that contained a beneficial regularity. This result was replicated using different stimuli and different regularities,...

Identifying people with a learning disability: an advanced search for general practice.
CONCLUSION: The Read Code search supported practices to populate their registers and was quick to run and review, making it a viable choice to support register revalidation. However, it did not find large numbers of people eligible for the LD register who were previously unidentified by their practice, suggesting that additional complementary methods are required to support practices to validate their registers. Analysis of the psychological impact of a vascular risk factor intervention: results from a cluster randomized controlled trial in Australian general practice
Conclusions: The reduction of K10 in the intervention group demonstrates that a general practice based intervention to identify and manage vascular risk factors did not adversely impact on the psychological distress of the participants. The impact of the intervention on distress was not mediated by a change in the behavioural risk factors or BMI, suggesting that there must be other mediators that might explain the positive impact of the intervention on emotional wellbeing.Trial registration: Australian New Zealand Clinical Trials Registry ACTRN12607000423415.

Impact of lifetime opioid exposure on arterial stiffness and vascular age: cross-sectional and longitudinal studies in men and women
Conclusions Data show that lifetime opioid exposure, an interactive cardiovascular risk factor, particularly in women, is related to linear, quadratic, cubic and quartic functions of treatment duration and is consistent with other literature of accelerated ageing in patients with OD.

Effect of Long-Term Vascular Care on Progression of Cerebrovascular Lesions Clinical Sciences
Conclusions—Nurse-led vascular care in hypertensive community-dwelling older persons did not diminish WMH accumulation over 3 years. However, our results do suggest this type of intervention could be effective in persons with high WMH volumes. There was no effect on lacunar infarcts incidence but numbers were low.Clinical Trial Information—URL: http://www.isrctn.com/ISRCTN29711771. Unique identifier: ISRCTN29711771.
